Requests can be made for semester-only courses. If you are in a year-long course, you may not drop the course at the semester; you are enrolled for the entire term of the course.

For all other requests: a) please complete the information requested below and submit the form and fee to Mrs. Toscano in the breezeway, or b) use the Mariner Mall to request a schedule change.

There is a $20 fee to request a schedule change.

Deadline to submit this form is Thursday, January 10, 2019. Fee $20.00

Your counselor will schedule an appointment with you. Appointments are scheduled based on date of receipt.
